Option 1:
Target Audience:

This intensive is appropriate for facility & community owners, corporate consultants & management, licensed nursing home administrators, adult foster care licensees, directors of nursing & nursing staff, licensed social workers, certified assisted living
directors, business office professionals and HCAM|MCAL associate partners.

Contact Hours:
This one-day intensive will provide participants the opportunity to earn up to 6 contact hours for LNHA, AFC/CALD. Participants will also have the opportunity to earn up to 6 nurse contact hours.
HCAM is an approved provider of continuing nursing education by the Wisconsin
Nurses Association, an accredited approver by the American Nurses Credentialing
Center’s Commission on Accreditation.

#1: Progressive Treatments/Approaches for Diabetes - Dr. Mark Jackson, Grand Traverse Pavilions
Explore the numerous new medications used for treatment of Diabetes.  Learn about the advantages and disadvantages of newer pharmacological approaches to managing Diabetes. Understand how these new approaches differ from our longstanding traditional approaches, how to monitor and how to help your residents maintain stable blood sugar control.

#2: Avoiding Adverse Drug Reactions and Achieving Remission From Depression & Generalized Anxiety - Rob Bright, Assurex Health
In this session, participants will leave armed with knowledge to reduce 98% of adverse drug reaction their residents experience today. Things like poor behavior, withdrawal, sadness, anger and dizziness. Genetics is the key to better medication selection in terms of dosage and medication mechanism of action. Your residents DO NOT have to live in a world where depression impacts their quality of life. This session will both improve and save lives.

#3: Good Bugs vs. Bad: Fecal Microbial Transplantation - Bill Vaughan, Remedi SeniorCare
Studies reveal that two-thirds of healthcare related Clostridium difficile infections occur in adults 65 years of age or older. Mortality in this population is significant with approximately 10% of elderly residents dying within 30 days of diagnosis. While drug therapy is the mainstay of treatment, it is not universally effective. This presentation will explore the indications, risks, and benefits of fecal microbial transplantation (FMT) for the treatment of C. Diff colitis. The clinical management of a geriatric resident undergoing FMT will also be addressed.

#4: The SNF Leader’s Guide to Occupancy & Mix: Addressing Current Challenges & Preparing for the Next BIG One in 2019! - Lou Ann Brubaker, Brubaker Consulting
The days when Admission & Marketing teams in SNFs alone could be the primary drivers in generating referral streams quickly are dwindling. This unique session addresses the IMMEDIATE actions that SNF leaders can take to enhance both current occupancy and mix AND ready their facility for ever acute care/payer expectations. Length of stay and outcomes leveraging will be discussed, as well what impact the upcoming Patient Driven Payment Model may have upon their facility. Participants will receive a proprietary outline developed by the presenter to use in crafting their outreach. Taught in engaging, straightforward, and non-theoretical approaches attendees will be able to enhance their current processes and identify opportunities to drive their future success.

LGBT & Aging: Providing Inclusive Care to LGBT Older Adults Presenters: Angie Perone & Judy Lewis
Description: Lesbian, gay, bisexual and transgender (LGBT) older adults have unique physical and mental health needs, and a history of trauma related
to LGBT identity, which makes it more difficult for older adults to trust health and human service providers. Increasing health, economic and housing disparities have further underscored the need for culturally responsive care. Together we will explore key issues, needs and challenges for serving LGBT older adults, share best practices and build skills for providing inclusive care to LGBT older adults.

After your registration form has been processed, an event confirmation will be emailed to your attention. Please be sure to include a current email addresss on this form. If you do not receive a confirmation within 14 business days of submitting your registration form to our office, please contact Kisti Boatright via e-mail at KistiBoatright@hcam.org.
Payment & Applicable Rate
All registration fees are due at the time of registration. We accept Visa, MasterCard, American Express or Discover. We will also accept a check or money order payable to HCAM. Incorrect rates will be adjusted to reflect the correct applicable rate and automatically charged accordingly. Cancellation & Refund Policy
All refund requests must be made in writing by e-mail to the attention of Kisti Boatright at KistiBoatright@hcam Telephone cancellations will NOT be accepted. Individual registration fees, less a 25% processing fee, will be refunded for cancellations received in writing prior to Oct. 26. Cancellations received after Oct. 26 and no-show registrants, will not receive a refund nor a credit to a future program.
Substitutions
Individuals registered to attend this program, but unable to attend, may send an alternate in their place. Please contact Kisti Boatright via e-mail at KistiBoatright@hcam.org with the first and last name, title and facility/company of both you and your substitute to process this request.
Dietary Restrictions/Special Accommodations
Individuals with special dietary restrictions or who require special accommodations to fully participate in this program, should contact HCAM in writing by e-mail to KistiBoatright@hcam.org no later than Oct. 26, detailing their need.
